When the user is Terminated from EC, User Offboarding activity gets created in Offboarding Program and Data Review steps gets assigned to Hiring Manager as well.
Post completion of Data review, Terminated User need to get the Offboarding task like below.
But these tasks is not presented to Terminated User.

Reproducing the Issue
1) Login to Instance
2) Terminate the user
3) Complete the Data review step for User as HM
4) Login as Terminated user and check the Home page. you will not see the Offboarding Tasks.
5) Again Login as Hiring Manager and check the Dashboard for User Activity.You notice Activity in Completed status.
Incorrect creation of Business rule for Offboarding Tasks
Please go to BPE monitor Process and see if there are any Unfinished Process.
If yes-> Check the Error ID and Provide to Support via Incident.
Also Please check the below rule.
There should be only one rule with rule scenario Select Offboarding Task Config but you might have multiple rule on scenario Select Offboarding Task Config.
So try to keep only one rule and re-trriger,OFB tasks will created.
Also verify the below steps.
1)verify whether configurations and RBP is maintained in client system.
2)Go to admin centre–BPE :Monitor Process and check whether Offboarding call is happened.if offboarding call is not happened then there is an issue in rules where system is not able to subscribe event.Please cross verify your rules.
3)Go to admin centre-Manage data-Process object and check whether offboarding activity is triggered.
4.Check BPE error tile in Homepage of manager to check error message.
