You have converted a lead, but you see that conversion tab has no details.

Reproducing the Issue
Go to Sales Workcenter
- Go to Leads View
- Search for Lead ABC (where ABC is the id of the lead which is not converted)
- Open lead ABC
- Select Convert to Account and Contact or Convert to Opportunity from Actions button
- Go to Conversion tab of lead ABC
Here you will see that conversion details section is empty
If the conversion happens in the lead for an active account and contact which is selected from duplicated list, then the account, contact information will be empty.
This is the expected system design because, system considers the active state of the selected account and treats that there is no new conversion which happened.
