You have checked the option "Create appointments in Microsoft Outlook as visits by default (users can change as necessary)" under Groupware Add-In Settings, however when you go in Microsoft Outlook and create a new appointment the Visit checkbox is not checked by default and user has to do this manually.

Reproducing the Issue
In Microsoft Outlook:
Click on New Items.
- Click on Appointments.
- Visit checkbox will be unchecked by default.
Once you define new settings for the Outlook Add-In, end users may need to reset their Add-In configuration to reflect the new settings immediately.
On Microsoft Outlook Add-In proceed as follows:
Click on Settings on the SAP Cloud For Customer Add-In options.
- Click on the button Reset.
- Press OK when the application asks for confirmation if you want to go back to default settings set by administrator.
