When the final approver clicks on "Approve", the page keeps on loading for a long time and then the message appears "The request has been approved".
But if we check, the workflow is still pending.

A custom string field is part of the data change that triggered the workflow. More than 256 characters were entered in this field. If the field's visibility is "Edit", the validation happens and the workflow does not get triggered. But when the field's visibility is "View", the validation does not happen when triggering the workflow.
If possible, use "Notes" field instead of the custom string field. If you cannot use “Notes” field and insist to use a custom string field only, then we have the limitation that custom string field can store only 256 chars.
