While creating a new user account in LMS we receive a validation error that states:
You must correct the following error(s) before proceeding:
- The user (user_name) already exists. Please create another one. You can configure user IDs so that they are not case sensitive
However, this user cannot be found anywhere in the system. Searching all active and inactive Users returns no results.

Reproducing the Issue
- Navigate to Learning Administration > Users > Add New > Enter the details
- Validation error is generated
This issue happens if as an admin you allow users to create their own accounts within "User-Created Accounts" along with the 'Approval require' option.
The validation message will be thrown if the User id exists in either the PA_STUDENT or PA_STUD_SELF_REGISTRATION (populated during self registration) database table.
Navigate to Learning Administration > Users > Account Requests > Search for the user ID in question and you will see this user.
As per your requirement you can either deny the user account or approve the user account.
