An Ad Hoc Report is not giving you Participant Information for your 360 Form, when those forms are in the Evaluation Stage or Completed.

Reproducing the Issue
1. Go to Analytics > Reporting > Ad Hoc Reports
2. Create an Ad Hoc Report for the 360 Multirater Domain
3. Include Columns like Participant First Name or Last Name
4. Generate the Report
5. Participant First Name and Last Name are not populated, or only populated for forms in a Modify Step.
This issue is most likely caused by enabling the option "Enabled anonymity in the 360 reporting functions" in Manage Templates.
This option is supposed to obscure, who is giving what rating in the 360 form and Ad Hoc is respecting this decision.
In a Modify Step you can still decide who will be ask to rate a subject, but after the form is send out for Evaluation all the participant names are removed from the form and from the Ad Hoc report.
The only way to report on the participant names after sending the form out for Evaluation is so disable the option "Enabled anonymity in the 360 reporting functions".
