What is that little chat box in the worksheets in the side of the employees' names?
Those are the Salary Notes: they can be modified by the current planner of the form in order to provide any kind of notes to the specific employee.

Reproducing the Issue
If the forms don't have the Salary Notes field enabled, how can this be done?
Go to the specific template > Plan Setup > Design Worksheet > Add Column > Standard
Select the field Salary Notes > click on Done
When you add it, this field will be automatically placed in the first fields ordering
Launch a form and you'll now see the chart boxes.
To add a note just click on it and a popup screen will appear
