A User self-assigns an Item to their Learning Plan and then registers for an offering of the Item.
The User then withdraws from the scheduled offering and message that says 'Do you want to remove the associated item from the Learning Plan?'.
When the User responds 'Yes' to remove the Item is not removed but rather stays on the User's Learning Plan.

Reproducing the Issue
- Create Item and SO
- Add both to a catalog
- Assign catalog to User (Assignment Profile)
- User self-assigns Item and registers for SO
- Remove/unregister SO
- Prompt: Do you want to remove the associated item from the Learning Plan?
- Item is still on Learning Plan
