2997321 - Column header is missing when a dimension is set to display both key and text

Symptom

When a dimension/characteristic is set to display both Key and Text (or Text and Key, the order is not relevant), the header is missing in the second colum, as shown in the screenshot below:

The "Repeat Members" option does not change the behavior.
