When an item (online, other, scheduled only, and scheduled and online) is inserted in the system and the "Item Summary Data Audit CSV" report is generated, it is possible to see there are three lines mentioning "Insert".

When an item is inserted, there is an insert to the item’s title, one for the item’s description and one for the item’s actual insert.
When the above action happens, the query will create new rows for the item’ title and description for different locales too.
The query has been implemented is such a way to logically work like this with the audit tables and this is the reason it brings more than one line in the report.
It means that the report query/result is not wrong, but they are working as the current design and it has been validated by the Learning Product Management team.
