It's desired to include extra fields to the Record Learning form.

SAP SuccessFactors Learning

The form is hard coded and does not support the inclusion of extra fields.

The enhancement ideas have been already submitted to have a customizable form, so additional fields can be included. Please refer to the links under See Also section and feel free to vote.
