When setting the Comments field from Items as Required, it is not enforced by the system. The * symbol is put next to it in the Item creation screen, indicating that it is required/mandatory field, however the Admin is able to create an Item without filling or populating it. It is also not indicated as required by the validation text to continue as explained in the replication steps.

Reproducing the Issue
Go to System Admin->Configuration->Record COnfiguration;
Search for Comments and set it as Required;
Go to Learning->Items;
Click in Add New;
Click to Continue, a validation message for the required field will appear but the Comments field will not be mentioned;
Fill all Required fields except the Comments;
Continue and create the Item;
It will allow to create the Item without filling the "Required" field of Comments.
Engineering team confirmed it as a limitation of the system and is working for the fix.
A code fix is targeted to be applied in version b1808 (LRN-37668).
