A couple symptoms may arise for this issue:
Primary indices are missing after homogeneous system copy
A homogeneous system copy with R3load (where the source and target database are Oracle), was carried out, this can be for example a migration with unicode conversion.
All indices were created by R3load (Migration Monitor) successfully with a valid time stamp according to the package-logs, for example:
(DB) INFO: /1BS/TEST_T_FK~0 created #20161024134447
- in the target system several thousand primary indices are missing in the database
- possibly also there are errors in SWPM post-installations steps, the report "RADDBDIF" fails, for example:
ERROR <timestamp> (root/sapinst) [CSiStepExecute.cpp:1187] id=controller.stepExecuted errno=FCO-00011
The step runRADDBDIF with step key |NW_ABAP_OneHost|ind|ind|ind|ind|0|0|NW_Onehost_System|ind|ind|ind|ind|onehost|0|

Creating indexes through process chains may fail with the error below, considering SAPSR3 is the schema owner:
Error message: ORA-01920: user * 'SAPSR3' does not exist" in Creating Index step of Process chain.
